Why Swelling & Bruising Happen After Back Lipo
You may be excited about your back lipo procedure, but your body views it as a traumatic experience. This is why you develop swelling and bruising afterward. Bruising is easy to understand. Surgery damages tiny blood vessels, leaving blood to pool in the area until the body mops it up. When you see a bruise, you’re seeing this pooled blood.
Swelling is a bit more complex. When your body detects injury to your back (or any) tissue, it sends extra blood, proteins, and immune cells to the area to promote healing. Though necessary, this influx of goodies causes a sort of traffic jam in your body as it experiences more volume than normal. The area expands, or swells, as these extras strain the area. Disturbances in the lymphatic system also contribute to swelling as lymph accumulates in the area due to disruptions in the vessels and nodes that allow for normal drainage.
Swelling begins almost immediately after surgery, taking only hours to develop. Bruising joins the party a bit later, showing up 24 to 48 hours after the procedure. Both resolve as you heal but tend to get worse before they get better.
Why Swelling on the Back Can Feel Different
Although a normal part of any surgery, swelling after back lipo is often a bit more intense. One reason is that back tissues are thicker and more fibrous than many other parts of the body. This makes swelling there feel firmer. Different people experience different firmness levels after back lipo, but you can expect it to be more pronounced than it is after liposuction in other areas.
The back is also unique in that it participates in so many of your movements. Sitting, standing, bending, and twisting all require participation from your back. It’s always on the job, and all that work can exacerbate swelling. Conversely, too little movement can impede fluid flow in the body and exacerbate back swelling. Think of your back like Goldilocks — too much movement will make your back swell, and so will too little. The trick is to find the balance that’s just right.
Bras, waistbands, and other clothing can also contribute to back swelling when you’re up and around again. The specific impact of clothing largely depends where on your back lipo was performed. Logically, after removing a bra bulge in the middle back, wearing a bra may irritate you a bit. Waistbands, of course, are a bigger concern after lower back or flank lipo.
What’s Normal vs. Not Normal With Swelling & Bruising
It’s easy to grasp the concepts of swelling and bruising as normal in the abstract, but things can feel a bit different when you’re actually experiencing them. You may have questions about whether something your body does is okay or not, and we encourage you to ask them. Generally, however, the following are perfectly benign:
- Bruises that transition from purple to green to yellow over about three weeks, and sometimes a bit longer
- Swelling that fluctuates throughout the day
- Uneven swelling that can persist for several weeks
- Firmness and lumpiness under the skin
- A feeling of tightness across your back
- Sensitivity, tingling, or numbness as nerves regenerate
Sometimes, however, our bodies like to do weird things. Although rare, complications can arise after back lipo. Contact your plastic surgeon for a check-in to be safe if you experience:
- Rapid or severe swelling, especially on one side
- Hot, red, or painful areas
- Unexplained fever or chills
- Pus-like or foul-smelling drainage from your incisions
- Noticeable fluid pockets (possible seroma)
- Worsening rather than improving pain
How to Reduce Swelling After Back Lipo
You can’t completely prevent swelling after back lipo. That’s just a fact. But you can take steps to minimize and reduce it, and we encourage you to do so. After your surgery, make sure you:
- Wear compression garments exactly as your surgeon instructs
- Take frequent, short walks to get your blood and other bodily fluids moving
- Stay consistently hydrated
- Minimize your salt intake
- Elevate your upper body when lying down
- Avoid ice unless instructed otherwise by your surgeon
- Refrain from smoking and drinking alcohol, which impede healing
How to Help Bruising Fade After Back Lipo
Bruising typically fades in two or three weeks, but deep bruises can take longer to resolve. As is true for swelling, there are things you can do to help minimize bruising and help it go away more quickly:
- Wear your compression garment as directed
- Avoid blood thinners before and after surgery if you can
- Get lymphatic massages if and when recommended by your surgeon
- Take arnica or bromelain supplements if your surgeon approves
You may have noticed that wearing your compression garment is our top suggestion for both swelling and bruising. That didn’t happen by accident. Compression garments are vital to healing after liposuction, and not wearing them enough or properly is one of the biggest mistakes patients make. These garments help your body significantly during recovery, so please wear them as directed.
When Swelling Seems Like It’s Not Going Down
Patients often worry that their post-op swelling isn’t going down as quickly as it should. This is a common concern, and we encourage you to reach out if you’re worried. We want to assure you, however, that swelling takes weeks to fully resolve. Although the bulk of the swelling will ease in about a month, residual swelling after back lipo can take up to a year to fully dissipate.
Know too that swelling fluctuates from day to day and even hour to hour at times. You may be swollen in the morning after being inactive all night, see the swelling ease as you get up and moving, and then see it worsen again at night after being on the move all day. (Remember Goldilocks. Too much and too little movement both increase swelling.) Your back may also feel firm or a bit uneven.
Do reach out to your plastic surgeon for help if you experience the potential signs of complications listed above. Also reach out to us if swelling is making you uneasy. We would rather check something that isn’t a problem than miss something that is. We’re here to support you and happy to do so.
